EP!C is a local non-profit, established in 1950, committed to enriching the lives of adults with intellectual and developmental disabilities.  We are currently looking for an Site Monitor to work in our Community Options division.

Duties will include:

  1. To support and educate the philosophy of the agency mission/vision statement and current strategic plan.
  2. Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations.
  3. Provide OJT and CBTA training for Host Family Providers, DSPs, ensuring maintenance of professional certification.
  4. Monitor the delivery of services and supports as outlined in each individual's Personal Plan ensuring the activities and supports reflect individual preferences.
  5. Assist the Individuals in striving towards independence in accordance with goals and objectives in the Implementation Strategies.
  6. Assist in admission, transfers and discharge of individuals.
  7. Interact with Community Options, Host Family providers, EP!C staff, Host Family Individuals, family members, other professionals, and other relevant individuals in a professional manner.
  8. Follow and implement all agency and departmental protocols and procedures.
  9. Communicate regularly with the QIDP and other team members regarding progress toward outcomes, health and safety and other issues that may arise at the Host Family setting.
  10. Assure dignity, privacy and rights of participants are respected at all times.
  11. Complete documentation as required regarding individuals served, quality assurance, and home environment.
  12. As mandated reporters ensure all allegations abuse and neglect are reported as required.
  13. Participate in Host Family vacations, Power Team activities, fund raising and other events that are included in the Community Options program.
  14. Attend Community Options &/or Host meetings, in-services, as directed by the Community Option Coordinator.  
  15. Perform other duties as assigned.



  1. Minimum of High School diploma or equivalent (GED). 
  2. Three years experience working with people with developmental disabilities preferred.
  3. Valid Driver's License and automobile insurance.
  4. Must have the ability to lift fifty pounds.
  5. Must obtain and maintain DSP certification (training provided).
