Position Objective: The Host Home Relief Staff will follow Rule 50, Dept. of Human Services Rule 115, 116, 119, and Rule 120, and other relevant regulatory guidelines. The Host Home Relief Staff will provide relief hours and vacation to Host Home Providers (HFP) on a routing basis as designated by Host Home Program Manager.

General Responsibilities:

  1. Attend all in-service training, community outings, Host Home meetings, and individual Discovery Meetings designated by Host Home Relief Manager
  2. Assist in training of Host Home Providers and Host Home Relief Staff
  3. As applicable, obtain and maintain "authorized status" as designated by the RN by successfully competing medication training and by competently and accurately administering and supervising medications
  4. Perform all safety and emergency procedures in a calm , accurate and professional manner
  5. When necessary, assist individuals with personal needs such as transferring, toileting, eating and grooming
  6. Delivers services and supports as outlined in everyone's Person-Centered Plan ensuring that activities and supports reflect individual preferences.
  7. Communicates regularly with the Host Home manager, QIDP, nursing, and other team members regarding progress towards outcomes, health and safety, and other issues that may arise.
  8. Assists individuals in maintaining their home, reporting needed repairs or modifications to the appropriate parties within established time frames.
  9. Recognizes individual differences and adapts teaching/ interacting style to best support the person's needs.
  10. Advocates for individuals through knowledge of personal references, protection from abuse and neglect or other rights violations, and supporting the development of personal relationships.
  11. Follow and implement all pertinent agency policies, departmental protocols, and procedures.
  12. Provide required documentation regarding program delivery, medication administration, financial records, and all other requested documentation
  13. Assure that the basic civil, human and legal rights of all persons are respected
  14. Supervise the individuals in community and at home throughout assigned shift
  15. Assure dignity, privacy and rights of program participants are observed at all times
  16. As a mandated reporter, ensure allegations of abuse, neglect, and exploitation are reported as required by EP!C policy and government regulation
  17. Provide needed transportation to people supported using an EP!C vehicle or their vehicle as required.
  18. Assist the individuals in striving toward independence in accordance with goals and objectives in the Personal Plan on the individualized Service Plan
  19. Act as a positive role model for individuals at all times
  20. Provide natural learning opportunities to teach independent living skills in accordance with the principles of normalization
  21. Involve the individual with a variety of in-home and community leisure activities in accordance with the principles of age-appropriateness and normalization
  22. Maintain documentation on all client structure programs and goals on specified data collection forms at the frequency indicated on the ISP in accordance with pertinent CILA and agency guidelines
  23. Provide required documentation regarding program delivery, medication administration, financial records and all other requested documentation
  24. Maintain progress note documentation in the areas of basic living skills, leisure activities and general progress as required
  25. Complete medication sheets as self-administration of client medications is supervised
  26. Maintain accurate financial records by retaining all receipts of bank transactions and expenditures
  27. Maintain other necessary documentation (e.g. mileage sheets, GERs, etc)
  28. Practice safety procedures and maintain acceptable standards of personal hygiene
  29. Report any concerns of the home environment (inside and outside) to assure the home is safe, clean and in orderly fashion.
  30. Request information, instructions and guidance form Host Home Program Manager in order that effective program service delivery may be maintained.
  31. Interact with all of Host Home individuals, EP!C staff/providers individuals from monitoring agencies/committees, parents, home members, other professionals and other relevant individuals in a professional manner

Marginal Responsibilities

  1. Perform other duties as assigned.
  2. As a mandated reporter, ensure allegations of abuse, neglect, and exploitation are reported as required by EP!C policy and government regulation

Education/Experience Requirements

  1. Must be a minimum of 18 years of age
  2. High School Diploma or equivalent (GED).
  3. Successfully complete CPR, First Aid, Infection Control, and Crisis Prevention Institute (CPI) training and maintain these certifications.
  4. Must complete DSP training in accordance with the state regulations and maintain acceptable status on the Health Care Worker's Registry under the Healthcare Worker's Background Check Act.
  5. Must obtain/maintain standing as an Authorized Direct Support Person for medication administration.
  6. Possess a valid driver's license and automobile insurance.
  7. Have access to an automobile
  8. Have access to a telephone
  9. Must be able to perform CPR/First Aid
  10. Must be able to pass the criminal background check

  9. Must possess emotional stability to handle stressful situations on a regular basis.
  10. Working knowledge of the principles of Person-Centered Planning and the Council of Quality Leadership (CQL).
  11. Will be required to drive a motor vehicle (EP!C's and employee's) as part of his/her employment responsibilities with EP!C. A valid Illinois Driver's License and insurance is required and must be maintained at all times while employed by EP!C.
  12. To perform job successfully the individual must be able to perform each essential duty satisfactory and have a flexibility to work evenings and weekends as necessary.

Physical Requirements

  1. Frequent walking and sitting.
  2. Regular reaching, grasping, and standing.
  3. Must be able to assist in the evacuation of residents in emergencies.
  4. Other physical activities related to an office environment.
  5. Operates general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer.
  6. Ability to lift 50 lbs.

  1. Work may occur in the following settings: Home, Office, Classroom, Site, Group Homes, or Community.
  2. Be a team player with a positive attitude towards the people EP!C supports, peers, EP!C staff and outside contacts.
